“Excessive sweating, less appetite, wt.loss, palpitations – on-of, Nervousness/startling reaction; Menstrual H/o: 

 

13.              The insured died on 24.07.2010 at Omni Hospital, Hyderabad due to cardio-respiratory arrest and severe anemia. The appellant addressed letter dated 28.01.2011 requesting the respondent to submit medical record of his wife for the treatment taken for epilepsy. 

“With reference to the subject cited above, I am to state that the deceased life does not suffer from Epilepsy at any time during her life time and we have not mentioned anywhere in the claim form as stated by you.
