2. Star Union Dai-Ichi Life Insurance Company Limited through its authorized officer situated at 11^th floor, Vishwaroop I.T. Park, plot no.34, 35 & 38 Sector-30A of IIP Vasi Navi Mumbai-400703.

                   Complaint under section 12 of the Consumer Protection Act.
