ORDER

Heard both sides. The complaint is disposed of in terms that Opposite party shall issue claim form to complainant within 10 days and thereafter complainant as guardian of nominee shall submit claim form with death certificate to OP and OP shall consider the claim within two months and if OP does not consider. The complainant is permitted to file fresh complaint.
