1.         ICICI Prudential Life Insurance Co. Ltd., ICICI PRU Life Towers, 1089, Appasasaheb Marathe Marg, Prabhadevi, Mumbai-400025 through its Managing Director.

2.         ICICI Prudential Life Insurance Co. Ltd., First Floor, SCO 9 to 11, Sector 9, Madhya Marg, Chandigarh, through its Manager.
