Seema aged about 41 years wife of late Shri Suresh Kumar, resident of House no.177-B, Ward no.1, Katju Nagar, Taraori, District Karnal. Aadhar no.842836721667.

1.Allahabad Bank, Branch Sonkra Road, Taraori, District Karnal, through its Bank Manager.

2. The Deputy General Manager, Allahabad Bank, Zonal Office: 2^nd floor, Bank square, Sector-17-B, Chandigarh.

3. Life Insurance Corporation of India, (PMJJBY Policy no.90010032) shop no.1 and 2, opposite Super Mall, sector-12, Karnal, through its Senior Manager/Chief Administrative Officer/Authorized Signatory.

                Complaint under section 12 of the Consumer Protection Act.
